Change of Major
This form is used by currently enrolled students who wish to change majors. The student should contact their present Academic Advisor and request the change; signatures from academic departments are required.
Interdisciplinary Studies Declaration 2015-2016
This is the application for formal admission into the Interdisciplinary Studies major. Pre-major students that have established their full plan of coursework should fill out and submit this form before Senior year.
Students that have been formally accepted into the major and registered for their final semester of coures should complete and submit this form to the Academic Advisor, along with receipt of Diploma Application Fee.
