Organizational mission or general goals:
Elder Advocates of SC, Inc. is dedicated to managing the finest in medical and ancillary services to the elderly and their families. We focus on our client with an emphasis on blending family, healthcare professionals and the network of resources in the community for successful aging and independent living.
Primary services provided:
On-site medical management; comprehensive client assessment to include medical social and environmental factors; scheduled home visits; telephone contact and communication with all service providers; facility assessment, facility nursing assessment of client and/or facility search; assist in full transition to alternative living environment; nutritional and medication education, quality resources, and on-going management; comprehensive safety evaluation of home environment; coordinate access to Medical Professionals such as gerontologists, podiatrists, eye & hearing specialists; accompany clients to medical appointments to insure appropriate medical information is being communicated to healthcare professionals; coordination of transportation for all needs; coordination of ancillary needs (i.e. home health aides, pharmacy, shopping, cleaning, and home maintenance); act as a liaison between the client, healthcare professionals and family members; assist with urgent care needs, hospitalizations, and discharges; provide resources for financial and legal concerns for the client and family; 24/7 contact for client and/or family members; customized services for clients, dependent upon their needs.
Primary clientele served:
Persons over the age of 55; persons in the midst of a healthcare crisis desiring return to active lifestyle; persons who desire a plan of care for continuing independence during the aging process; out of area/state family with elderly relatives residing in Horry and Georgetown Counties.

All employees of Elder Advocates of SC, Inc. are registered Nurses (licensed in SC), Certified Case Managers, members of the Geriatric Care Managers Association.

Elder Advocates of SC, Inc. can serve up to 40 clients in its current configuration.

Elder Advocates of SC, Inc. is fully licensed and insured. HIPPA compliant. SC licensure for Registered Nurses, National Certification for Case Managers (CCM), American Academy of Case Managers, Geriatric Care Managers Association, SC Notary Public.
