Organizational mission or general goals:
To provide comprehensive, compassionate counseling and care management to Grand Strand Seniors.
Primary services provided:
We provide multi disciplinary services by MSW/LISWs, RNs and spiritual counselors. Each is a fee for service basis. Counseling covered by Medicare and other insurances. Mental health/counseling service, care management, consultations, telephone reassurance program, guardianship
Primary clientele served:
Persons aged 60 +, regardless of ability to pay.
Employees and/or volunteers:
One full time, several part time employees (RNs, Chaplains)
Staff members eligible for professional credentialing and credentialed staff members employed:
Victoria Carey - MSW, LISW-CP (Licensed Independent Clinical Social Worker). All of the nursing staff are Registered Nurses.
Clients/patients/participants served each year:
We are equipped to serve up to 50 clients at this time.
Organization accredited or certified by an outside agency or association:
Licensed Clinical Social Worker (LISW-CP) licensed by State of SC and by Council on Social Work Education. Member, National Guardianship Association. Member National Hospice-Palliative Care Organization. Notary Public-SC. Member National Association of Social Workers. Member National Professional Geriatric Care Managers.
Special services or pricing for senior citizens:
We assist seniors regardless of ability to pay. We accept medicare assignment.
Comments:
We are the only care management agency in the area that is (1)multi disciplinary, (2) spiritually based, (3) providing services regardless of ability to pay, (4)able to accept medicare payments (for counseling services), (5) able to provide mental health counseling in the seniors home- under medicare. |
