Organizational mission or general goals:
To provide quality hospice care to patients with a limited life expectancy, as well as support for their caregivers.
Primary services provided:
Hospice care is provided by an interdisciplinary team utilizing an approach to the care delivered to patients either in their own home or extended care facility. The Hospice Team includes the following members; Attending Physician, Medical Director, Registered Nurses, Certified Nursing Assistants, Social Worker, Spiritual Care Counselor, Bereavement Coordinator, Trained Volunteers, Dietician and other therapies.
Primary clientele served:
Heartland serves patients with a limited life expectancy (6 months or less). Heartland Hospice strives to provide care to all who are eligible and need hospice care irrespective of ability to pay.

23 employees and ~20 volunteers

Medical Director (1); Social Worker (MSW) (1); Director of Nursing (1); Registered Nurse Case Managers (5); Dietician (1); CNA (5); LPN (0)

Since our opening in February of 2007, we have grown to a census of 50. We continue to look forward and plan for continued growth as our reputation for excellent care and excellent customer service becomes more widely known. We are a part of a national company that provides hospice care to patients across the country. At Heartland, we help preserve our patients' dignity, show them the respect they richly deserve, and provide caring compassion.
