Organizational mission or general goals: The health care professionals at Advantage Hospice & Home Care turn homes into healing environments where patients and their families learn to care for themselves. Working hand in hand with the patient's physician, our skilled professionals offer superb and compassionate service. We offer skilled nursing, including home health aide and medical social services all within the comfort and security of the patient's home. Advantage Hospice & Home Care is dedicated to bringing together people who care with people who need care.
Primary services provided: ~ Specialized health care services that focus on pain relief, symptom control, comfort and support for terminally ill patients at home
~ Special program for skilled nursing facility residents and hospice patients in contract hospitals
~ Care for children facing life-threatening illness
~ An interdisciplinary team of specially trained professionals to help care for the patient including a physician, nurse and social worker as well as a chaplain, health aide, homemaker and volunteer as needed
~ 24-hour on-call availability of a hospice medical professional
~ Medical supplies and equipment
~ Instruction in care giving to help loved ones care for the patient
~ Assistance with the patient's personal hygiene
~ Respite care, support and practical assistance from trained hospice volunteers
~ Emotional and spiritual support for patients and their loved ones
~ Bereavement support for surviving family members, loved ones and care givers for a minimum of 13 months after the death of a patient
Primary clientele served: Hospice provides palliative care and support for persons with terminal illness and a prognosis of having six months or less to live. Approximately 46% are cancer patients with the balance suffering from AIDS, Alzheimer's disease, ALS (Lou Gehrig's disease), end stage heart and lung disease and other life-threatening non-cancer conditions. In recognition of the fact that it can be difficult to determine prognosis for patients affected by a life limiting, non-cancerous diagnosis, the National Hospice Organization created several local medical review policies. These objectively address symptoms presented in an effort to establish a prognosis. Please feel free to contact one of our trained professionals with any interpretive issues.
