Organizational mission or general goals:
Primary services provided:
Adults day care with active fitness program and therapy services. (Level I = $45/day; Level II = $55/day; Level III = $65/day. Therapy services are billed separately.)
Primary clientele served:
Individuals 18 years of age and older whyo are chronically ill or disabled, requiring varying levels of supervision and/or social stimulation.
Employees and/or volunteers:

Staff members eligible for professional credentialing and credentialed staff members employed:
We have a professionally trained staff of registered nurses, licensed practical nurses, certified nursing assistants, personal care assistants, van drivers, and therapists.
Clients/patients/participants served each year:
Approximately 40 clients per day, Monday through Friday.
Organization accredited or certified by an outside agency or association:
We are monitored by the S. C. Department of Health and Environmental Control, Community Long Term Care SC, Medicaid, and the Veterans Administration.
Special services or pricing for senior citizens:
We offer half day programming and pricing for seniors, and work to provide social services assistance and referrals to other appropriate community-based resources.
Comments:
We offer transportation to and from the center for clients living within 15 miles distance. Our Day Center offers physical and occupational therapy provided by therapists who are specially trained in working with dementia and physically challenged clients. |
