Organizational mission or general goals:
To restore, maintain, and promote the health status of individuals while caring for them in the comfort of their own homes.
Primary services provided:
Active Day of the Grand Strand provides medical adult day care services with a daily "Active Fitness" program. We offer nursing care for such services as medication management, blood sugar monitoring, tube feedings, wound care, catheter management, etc. Our therapeutic activity programing offers stimulating activities, community outings, entertainment, educational opportunities and much more.
Primary clientele served:
Those individuals who are 18 years and older with varying degrees of mental and/or physical impairment(s) with a need for supervision or socialization.
Employees and/or volunteers:

Staff members eligible for professional credentialing and credentialed staff members employed:
We have a professionally trained staff of registered nurses, certified nursing assistants, and personal care/activity assistants.
Clients/patients/participants served each year:
Approximately 45 clients per day, Monday through Friday.
Organization accredited or certified by an outside agency or association:
We are monitored by the South Carolina Department of Health and Environmental Control and Community Long Term Care Medcaid Waiver Services.
Special services or pricing for senior citizens:
We offer counseling for funding options such as grant and/or Medcaid application processes. We also serve as a resource for community-based services.
Comments:
We offer optional transportation services to and from the Center for clients who live within 15 miles distance, which is provided by an outside vendor. |
