Organizational mission or general goals:
To provide care and education to assist patients and their families is achieving the highest level of independence in their homes and communities.
Primary services provided:
Skilled nursing; physical therapy; occupational therapy; social work; home health aides. (May be billed to Medicare, Medicaid, insurance, or private pay.)
Primary clientele served:
All ages and physical conditions.
Employees and/or volunteers:
Staff members eligible for professional credentialing and credentialed staff members employed:
All professional staff eligible for licensing or certification hold those credentials.

N/A
Organization accredited or certified by an outside agency or association:
We are accredited by the Accreditation Commission for Health Care, and monitored by the S. C. Department of Health and Environmental Control (DHEC) and Medicare.
Special services or pricing for senior citizens:
We accept Medicare assignment; i.e, accept whatever Medicare pays with no further obligation to the patient.
Comments:
Home health services require an order from the patient's physician stating the need and a diagnosis. |
