Organizational mission or general goals:
To enhance the quality of life for individuals and families served by this practice.
Primary services provided:
Individual Psychotherapy ($110/hour)
Geriatric Assessment ($110 for evaluation)
Geriatric Care Management ($100/hour)
Consultation; Education; Expert Witness as a court visitor for Probate Court. (Medicare assignment accepted; private pay for home visits.)
Primary clientele served:
Adults over age 18, especially retirees in need of individual or family therapy or geriatric care management services.
Employees and/or volunteers:
One, although I contract out personal care and medical services to assist clients remaining in their homes.
Staff members eligible for professional credentialing and credentialed staff members employed:
Masters Degree in Social Work; medical care and health services training received at OARS-GET Clinic at Duke University.
Clients/patients/participants served each year:
Over 2,000 clients per year.
Organization accredited or certified by an outside agency or association:
Certified by the Academy of Certified Social Workers and licensed as an independent social worker in South Carolina.
Special services or pricing for senior citizens:
I accept Medicare assignments for office visits; private pay for home visits.
Comments:
I provide professional geriatric assessment, consultation to caregivers, families, bank trust officers, attorneys, and probate court regarding a treatment plan. I provide care management for psychiatric, medical, and geriatric clients. |
