2012 - 2013 Instructions on IRS Tax Return Transcript

What is an IRS Tax Return Transcript?

A Tax Return Transcript shows most line items from your tax return (Form 1040, 1040A or 1040EZ) as it was originally filed, including any accompanying forms and schedules. In most cases, your transcript includes all the information a lender or government agency needs. It does not show any changes you, your representative or the Internal Revenue Service made after you filed.

How do I obtain it?

Tax filers can request transcript, free of charge, of their 2011 tax return from the IRS in one of the following three ways:

  • Online Request
    1. Available on the IRS website at www.irs.gov
    2. On the main page under Tools, Click "Order a Tax Return or Account Transcript"
    3. Click "Order a Transcript"
    4. Provide the tax filer's SSN, DOB, Street Address, and Zip Code (these must exactly match the data provided on your tax return).
    5. Click "Continue"
    6. In the "Type of Transcript" field, select "Return Transcript" and appropriate year requested in the "Tax Year" field
    7. Click "Continue"
  • Telephone Request
    1. Automated attendant - 1-800-908-9946
    2. Live person (not recommended) - 1-800-829-1040 - wait times can vary. You may be able to request that the transcript be faxed to you rather than be mailed.
  • Paper Request
    1. Complete Form 4506-T (Request for Transcript of Tax Return).
    2. Form 4506-T is available online at http://www.irs.gov/pub/irs-pdf/f4506t.pdf.
    3. Mail to the address indicated on the form.
