Every student entering a South Carolina college or university is required to provide an immunization record. All students attending Coastal Carolina University are required to meet the University's Immunization Requirements prior to registration; a copy of your immunization record must be provided to student health services prior to your enrollment. Please complete the Health History/Immunization Formor providean immunization record on a health care providers stationary that has been signed and verified by a physician, physician assistant, or nurse practitioner, or stamped by the health care facility and contain the month, day and year vaccine was given. All immunization and tuberculin skin test documents must be submitted in English. Required Immunizations:
M.M.R. (Measles, Mumps, Rubella)
(2 doses required at least 28 days apart for students born 1957 or later and all health sciences students).
dose given at age 12 months or later.
dose given at least 28 days after first dose.
Tuberculin skin test (PPD or Mantoux) within the past 12 months (international students from high risk countries). If there is a history of a positive skin test, a chest X-ray is required.
Residence hall students under 25 years of age must provide proof of meningococcal vaccine or sign the waiver/declination on the University Health History and Immunization Form.
Tetanus (DTap, DTP, DT, Td or Tdap) vaccination within the last ten years.
