This form has been approved by the AAC, PENDING the following edits:
(1) Amend the attached syllabus by:
(a) Copying the full contents of the “Catalog description” field on the form to the attached proposed syllabus, under the heading “Course Description”, so that you list the number of credits and eliminate the future tense which currently exists in the attached syllabus.
(b) Under “Grading” identify the number of assignments and their frequency throughout the semester (while the grading scale is adequately described, what it is based on isn’t).
(c) Identifying your attendance policy with an explicit separate heading (at present it appears under “Considerations”).
(2) Change the “On-going & Reallocated” number to “1” in the Full-time Faculty row under the Assessment heading near the bottom of the form.
