Accessibility and Disability Services

ADS Testing Center

The Testing Center is available to students registered with ADS and approved for testing accommodations. Students access the Testing Center and coordindate arrangements with their instructors and ADS via the use of the Accessibility and Disability Testing Center Proctor Sheet (pdf).

Steps for scheduling and taking an exam at ADS:

  1. Student obtains hard copy of Proctor Sheet at ADS or prints hard copy of the Accessibility and Disability Testing Center Proctor Sheet (pdf).
  2. Student completes top portion of the form with identifying information.
  3. Student provides Proctor Sheet to instructor, preferably during office hours, to complete the instructor portion of the form.
  4. Student submits completed Proctor Sheet to ADS at least five days prior to the exam. 
  5. Student schedules testing time with Testing Coordinator at ADS. When scheduling tests, please keep in mind that testing is initiated between 8:30 a.m. to 3 p.m., Monday through Friday. (If you have a test that will take more than two hours to complete, it cannot be scheduled at 3 p.m.). Students planning to take final exams in the Testing Center must inform Accessibility and Disability Services two weeks prior to finals.
  6. On the day of a scheduled test or exam, arrive at Kearns Hall 106 at the scheduled time. The allotted test time will not be extended due to tardiness. Students arriving early will not be permitted to start the test until the scheduled time, nor will students be permitted to study during the scheduled testing time. Instructors will be notified of late arrivals and no shows.
  7. Students accommodated with testing services at the Testing Center are expected to adhere to the  as stated by the University. Please be advised that the testing facilities at Accessibility and Disability Services are monitored and recorded by closed-circuit television.

Information for Instructors

  • After the Proctor Sheet has been filled out and returned to Accessibility and Disability Services by the student, the professor must email or campus mail the test to the Testing Center Coordinator (106 Kearns Hall) prior to the scheduled test day.
  • On the day of the exam, the student will start the test at the time given by the instructor. They will not be allowed to begin early. The instructor will be notified of late arrivals and no shows and time allotted for the test will not be extended due to tardiness.
  • Once the test is completed, the test will be returned to the instructor. The proctor sheet will tell the Accessibility and Disability Services staff how the instructor would prefer to receive the completed exam.
  • Students accommodated with the Testing Center at Accessibility and Disability Services are expected to adhere to the Academic Honesty Policy as stated by the University. The testing center is monitored by ADS staff. Potential violations of the and the will be referred to the appropriate offices and processes. 
  • Due to space and personnel constraints, students may arrange accommodations directly with instructors once Faculty Notification Forms have been shared. Instructors are permitted to administer tests and arrange for accommodations.
