Documentation guidelines are provided to assist students and professionals in submitting appropriate documentation of the disability. Assessments for documentation are expected to have been completed in recent years (generally within 3 years). While IEPs and 504 Plans are helpful information, they do not constitute sufficient documentation unless they include comprehensive assessment results. Notes from physicians written on prescription pads will not be accepted. Approximately one week after submitting documentation, students may call the Office of Accessibility and Disability Services to ensure that the documentation received is adequate.
