Pre-Visit Forms

You are encouraged to complete all applicable pre-visit forms prior to visiting Student Health Services. In order to complete pre-visit forms you will need to click on the link below and create a unique username and password if you have not done so already and complete the applicable pre-visit form. 

The Health History pre-visit form may also be found underneath the pre-visit form section; however, it should only be completed one time every academic year.  Also, the Authorization for the Release of Protected Health form (Authorization to Disclose Information form) maybe found in the same section; but it should only be completed when you are requesting records to be released to/from Student Health Services to/from another party (i.e. Healthcare provider, self, parent, school, etc.).
